Dr. Greggory Hurlbut M.D.

15Dr. Hurlbut is originally from Western Nebraska. He received a Bachelors’ Degree in Chemical Engineering in 1977 from the University of Nebraska Lincoln. In 1980 he received his Doctor of Medicine from the University Of Nebraska Medical Center College Of Medicine in Omaha; completing his residency in family medicine in 1983. After completing his residency Dr. Hurlbut joined SLFP and became a partner in the practice.
Dr. Hurlbut and his wife, Sheri, have two sons.

Dr. Hurlbut currently has privileges with both St Elizabeth Hospital and Bryan Health, and is accepting new patients with most major commercial insurance plans.

Dr. Brandon D. Webb M.D.

24Dr. Webb graduated from Baylor University in 1992 with a Bachelor of Science degree. He received his Doctor of Medicine degree from the University of Texas at Houston Medical School in 1996.

After serving as chief resident and completing his residency at Memorial Hermann Hospital Southwest in Houston, Texas in 1999 he moved to Lincoln Nebraska to join Primary Care Partners. He has since been a partner at SLFP. Dr. Webb is married to Dr. Kristin Webb and has three daughters and one son in elementary and middle school. Outside of medicine, Dr. Webb has interests in running, camping, and concert promotion.

Dr. Webb currently has privileges at St. Elizabeth Hospital and Bryan Health System in Lincoln, Ne. He is board certified in Family Medicine and holds medical licenses in Nebraska and Texas.

Dr. Nathan DeNell D.O.

13Originally from Lincoln Ne. Dr. DeNell received his Bachelors Of Science from the University of Nebraska Lincoln in 1998. Dr. DeNell then received a Master’s of Science from Barry University in Miami FL in 2001. Following his Masters Degree Dr. DeNell received his Doctor of Osteopathic Medicine in 2005 from Nova Southeastern University in Ft. Lauderdale FL. Dr. DeNell completed the rotating Internship at Arrowhead Regional Medical Center, Colton CA in 2006 before completing his Residency in Family Medicine from Lincoln Medical Education Partnership; Lincoln Ne in 2009. From 2009-2012 Dr. DeNell was a staff physician at Stevens Creek Family Medicine. Dr. DeNell joined Primary Care Partners January 2012 and is now a partner in the practice.

Dr. DeNell and his wife, Kerri, have one son and three daughters.

Dr. DeNell currently has privileges at both St. Elizabeth Hospital and Bryan Health.

Dr. John Majerus M.D.

43Originally from Lincoln Ne., Dr. Majerus graduated with his Bachelor of Science from the University of Nebraska Lincoln in 1994. In 1998 Dr. Majerus received his Doctor of Medicine from the University of Nebraska Medical Center in Omaha. Dr. Majerus completed the Lincoln Family Medicine Residency Program in 2001. Following residency Dr. Majerus went to work at Holmes Lake Family Health Center until 2011. From 2011-November 2013 Dr. Majerus practiced at Fallbrook Family Health Center. Dr. Majerus has been a member of Primary Care Partners since November 2013. Dr. John Majerus also served as a flight surgeon in the Air National Guard for 22 years retiring in 2012. Dr. Majerus enjoys spending time with family, running and outdoor activities.

Dr. Majerus currently has privileges at both Bryan Health and St Elizabeth Hospital.

 Dr. Rachel Blake M.D.

21Dr. Blake has practiced at Primary Care Partners since January 2016. She earned her medical degree at the University of Nebraska Medical Center in 2002 and completed her family practice residency program at Lincoln Family Medical Program in 2005. She became board certified by the American Board of Family Physicians in 2005. Dr. Blake has previous experience through the Nebraska Army National Guard including a deployment to Iraq in 2005 and Valley County Health Systems in Ord, Nebraska, also worked for Women’s Clinic of Lincoln just prior to joining Primary Care Partners.  Her clinical interests include providing care for all members of the family and Military Medicine. Dr. Blake enjoys being outdoors, camping, hiking, biking and playing with her children.

Dr. Blake currently has privileges at both Bryan Health and St. Elizabeth Hospital.

Dr. Susan Johnson M.D.


Dr. Johnson has practiced at Primary Care Partners since August of 2016. She was raised in Omaha and earned her medical degree at the University of Nebraska Medical Center in 1982.After completing her residency in family medicine at Texas Tech University she joined the faculty at Texas Tech, where she coordinated family practice education for residents and students. Dr. Johnson is certified by the American Board of Family Medicine. She moved to Lincoln in 1988 and began practicing family medicine. Since moving to Lincoln, Dr. Johnson and her family have been active in church and community activities. In her spare time, she enjoys traveling, music and gourmet cooking.

Dr. Jaime Dodge M.D.

Dr. Jaime Dodge is a 2002 graduate of the University of Nebraska College of Medicine and a 2005 graduate of the Lincoln Family Medicine Residency Program. He has eleven years of full-spectrum Family Medicine practice experience including rural practice in Alliance Nebraska from 2005 to 2008, and Ida Grove, Iowa from 2008 to 2011. He later trained Family Medicine physicians at the Siouxland Medical Education Foundation Family Medicine residency program in Sioux City, Iowa from 2011-2016 where he held clinical professor appointments from the University of Iowa and South Dakota, schools of medicine and was voted Faculty Physician of the year in 2015. Prior to joining Primary Care Partners, he served as a hospital physician at Bryan Medical Center in Lincoln. He is a certified Diplomat of the American Board of Family Medicine and a Fellow of the American Academy of Family Physicians and has practice interests in all aspects of Family Medicine including care of the whole family, office-based procedures and treatment of skin disease. Dr. Dodge served twelve years in the Army National Guard, including service in Iraq and Afghanistan. He is married with five children and enjoys spending time with them in church and participating in outdoor activities.

Angela Zavala D.N.P

Nurse Practitioner, Angela Zavala was raised on a farm near David City, Nebraska.  She graduated in 2003, from Bryan College of Health Sciences with her diploma in nursing, and completed a Bachelor of Science in Nursing in 2005, from the University of Nebraska Medical Center.  Having over fifteen years of nursing experience, she has worked in areas including adult and neonatal intensive care, as well as ambulatory surgery.  She received a Doctorate of Nursing Practice as a Family Nurse Practitioner in 2018 from Nebraska Methodist College.  She is a board-certified Family Nurse Practitioner by the American Academy of Nurse Practitioners.  She is a member of the American Association of Nurse Practitioners and the Sigma Theta Tau Honor Society of Nursing. Angela has been selected as one of the 2018 “40 under 40” Emerging Nurse Leader Honorees by the Nebraska Action Coalition. She is married with three children and enjoys reading and traveling in her free time.
